Step #2
Provide the clinic with the following information at the time of the consultation:
- Insurance Company Name, Address, Phone and Fax number
- Adjuster's Name
- Claim Number
- Referral to Lindora with goal weight included
(and medical necessity report, if available)
- Referring Physician, Phone and Fax
Step #3
Once Step 2 is complete, Lindora will file a claim with the workers' compensation insurance company.
